Change of Names

Procedure

Persons wishing to change their names from one name to another are required to produce the following:

• An Application for the Change of Name
• A sworn Affidavit in support of the Application
• Birth Certificate of the Applicant
• Identification of the Applicant
• Consent of the parent in certain circumstances

How much does it cost to change a name?

The cost of a Change of Name is two hundred ($200.00) dollars.

How long does it take to have a change of name?

A Change of Name takes between six (6) to eight (8) weeks
